Mopani; Sekhukhune; Waterberg and Vhembe.<\/p>\n<h3>ABOUT THE BURSARY PROGRAMME \u2013 FIELDS COVERED, COVERAGE VALUE &amp; SERVICE CONTRACT<\/h3>\n<p>The Limpopo Department of Health will be awarding bursaries each year, to deserving and financially needing students, within the\u00a0<strong>Medical\/ Health Science<\/strong>\u00a0field.<\/p>\n<p>The bursary will cover the following expenses: registration fees, tuition, prescribed textbook allowance, accommodation and meals.<\/p>\n<p>Once students have graduated, they will be required to enter into a Contractual Agreement to work for the Limpopo Department.<\/p>\n<h3>ELIGIBILITY REQUIREMENTS<\/h3>\n<p>Applicants must satisfy the following minimum entry criteria before applying (please note that failure to satisfy all the requirements will lead to your application not being considered):<\/p>\n<ul>\n<li>South African citizen<\/li>\n<li>Studying or intend on studying within one of the aforementioned fields<\/li>\n<li>Residing within the Limpopo Province<\/li>\n<li>From a previously disadvantaged background<\/li>\n<li>Proven financial need<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h3>HOW TO APPLY FOR THE BURSARY<\/h3>\n<p>Application forms can be downloaded from:\u00a0<strong>http:\/\/www.doh.limpopo.gov.za\/?s=bursary<\/strong><br \/>\nAlternatively, application forms can also be obtained from a Department of Health District Office (from one of the addresses below)<\/p>\n<p><em>*Please Note: application forms must be certified by a Commissioner of Oaths.<\/em><br \/>\nSubmit clear copies of the following supporting documentation along with your completed application form (the submission of these documents is compulsory \u2013 if any items are missing, your application be disregarded):<\/p>\n<ul>\n<li>ID document (certified copy)<\/li>\n<li>Matric certificate, if completed Matric (certified copy)<\/li>\n<li>Latest tertiary academic record, if a tertiary student (certified copy)<\/li>\n<li>Proof of acceptance at a tertiary institution<\/li>\n<li>Proof of residence (certified copy)<\/li>\n<li>Parents\/ guardians proof of income (latest payslips if employed\/ affidavit if unemployed\/ pension statements if on state pension\/ death certificate if deceased)<\/li>\n<li>Current Matric and teriary students must also submit their year end results, before the end of the first week of January.<\/li>\n<\/ul>\n<p>Applications must be posted to one of the following locations:<br \/>\nDepartment of Health<br \/>\nFidel Castro Ruz House<br \/>\n18 College Street<br \/>\nPolokwane<br \/>\nOR<\/p>\n<p>Department of Health District Offices:<\/p>\n<ul>\n<li>Capricorn: 34 Hans van Rensburg Street, POLOKWANE<\/li>\n<li>Mopani: Former Gazankulu Parliamentary Offices, GIYANI<\/li>\n<li>Sekhukhune: Old Parliamentary Building, LEBOWAKGOMO<\/li>\n<li>Waterberg: Corner Thabo Mbeki &amp;Elias Motswaledi Street, MODIMOLLE<\/li>\n<li>Vhembe: Old Parliamentary Buildings, Next to High Court, THOHOYANDOU<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h3>CLOSING DATE<\/h3>\n<p>30 November annually.<br \/>\n(Applications submitted after this date will not be accepted \u2013 please apply before the closing date to ensure your application is considered.)<\/p>\n<h3>CONTACT THE BURSARY PROVIDER<\/h3>\n<p>For any queries related to this bursary programme, please contact the Limpopo Department of Health directly:<br \/>\nTel: 015 293 6027<br \/>\nFax: 015 293 6211\/ 20<\/p><div class=\"forms-content\" id=\"forms-3629418922\"><div style=\"width: 300px;
