These include:<\/p>\n\n\n<ul class=\"wp-block-list\"><li>Two recent payslips<\/li><li>A bank statement<\/li><li>An original ID or passport or a certified copy of either one of these documents<\/li><li>Banking details and a completed&nbsp;<a href=\"http:\/\/www.labour.gov.za\/DOL\/documents\/forms\/unemployment-insurance-fund\/form-ui-2-8-application-to-pay-uif-benefits-into-banking-account\">form UI-2.8<\/a><\/li><li>A completed&nbsp;<a href=\"http:\/\/www.labour.gov.za\/DOL\/downloads\/documents\/forms\/unemployment-insurance-fund\/UI-2%207_2015.pdf\">UI-2.7 form<\/a>, completed by your employer, stating a record of all the funds you received as an employee during your time of employment<\/li><li>A completed form&nbsp;<a href=\"http:\/\/www.labour.gov.za\/downloads\/documents\/forms\/unemployment-insurance-fund\/Form%20UI2.3%20-%20Application%20for%20maternity%20benefits.doc\">UI-2.3<\/a>, which is the UIF Maternity Benefits application form<\/li><li>A medical certificate or your baby\u2019s birth certificate as proof that you actually had a baby<\/li><li>A completed&nbsp;<a href=\"http:\/\/www.labour.gov.za\/DOL\/downloads\/documents\/forms\/unemployment-insurance-fund\/UI-4_2015.pdf\">form UI-4<\/a>&nbsp;(follow up form), which ensures the continuation of payment of maternity benefits<\/li><\/ul>\n\n\n<h4 class=\"wp-block-heading\">Where to claim maternity benefits in South Africa<\/h4>\n\n\n<p>New moms will have to go to their&nbsp;<a href=\"http:\/\/www.labour.gov.za\/DOL\/contacts\/Labour%20Centres\">nearest labour centre<\/a>&nbsp;in order to submit the documentation before they can receive maternity benefits.<\/p>\n\n\n<p>You can expect compensation of between 38% and 58% of your gross monthly salary when applying for your UIF Maternity Benefits.\u00a0 New moms are entitled to between one and four month\u2019s salary for every six months they have worked.<\/p><div class=\"forms-content\" id=\"forms-2578933296\"><div style=\"width: 300px; padding: 20px; margin: 0 auto; border: 1px solid #ddd; border-radius: 8px; background-color: #f9f9f9; text-align: center; font-family: Arial, sans-serif;\">\r\n    <p style=\"font-size: 16px; margin: 0 0 10px;\">Need Information or Confused about Something ?
